The Paediatric Program is currently recruiting part-time Dietitian for the Scarborough Healthy Outcomes Program. The position will have responsibilities supporting HOPPS patients at SHN Centenary site and will also require visits to community centres, homes and schools. The HOPPS Dietitian will work closely with the members of multidisciplinary team to develop the core model of care for the HOPPS Program. In collaboration with the HOPPS Program team, you will facilitate the daily HOPPS Program operations. This includes overseeing all aspects of program implementation and delivery. You will play a key role in ensuring the highest standards in programming and delivery are maintained.

Responsibilities:

• Integrates clinical experience with strategic vision to participate in the development of the model of care for the HOPS program.
• Assists in program planning, implementation and evaluation.
• Develop and provide nutrition education sessions for children and their families.
• Responsible for the provision of appropriate, rational and cost efficient nutritional therapy to overweight and obese patients and their families in both individual and group settings
• Monitoring the effectiveness of nutrition intervention and care.
• Act as a resource to children and/or youth and families, other health care professionals and referral centres on nutrition education.
• Integrate research into practice and create/utilize and/or disseminate new knowledge.
• Conducts environmental assessments (home, community, and school) to help guide children and families through the treatment program.
• Assists with participant intake, screening and selection process.
• Orders, prepares, administers, scores and analyzes a battery of inventories and tests in conjunction with HOPPS team members.
• Assists with data entry, and ensuring all metrics and evaluation reports are completed as required.
• Supports tracking and reporting of participant progress and in the development of treatment plans.
• Develops a pre/post questionnaire for use at the onset/completion of the program components.
• Conducts follow-up calls to HOPPS program graduates and recording information gathered and recording information gathered for reporting purposes.
• Participates in developing and executing group education sessions with children, youth, and families. Creates, utilizes and/or disseminates new knowledge.
• Attending meetings as required.

Requirements:

• Baccalaureate degree in Nutrition.
• CNSC or a research Master′s Degree would be an asset.
• Registration with the College of Dietitians of Ontario and a member of Dietitians of Canada.
• Experience with counseling on obesity or involvement in a similar program would be an asset.
• Previous experience in pediatric nutrition is strongly preferred.
• Ability to collaborate effectively as a member of the health care team and take initiative and leadership when appropriate.
• Excellent interpersonal, communication and organizational skills and demonstrated ability to utilize critical thinking and problem solving skills.
• Attention to detail and efficient time management are required.
• Demonstrated ability to function as a member of the interdisciplinary health care team.
• Exposure to Lean principles, an understanding of methodologies and tools and a commitment to the use of these in support of providing the best healthcare experience possible to our patients
• Demonstrates understanding of and commitment to SHN's comprehensive safety programs and practices including staff safety, patient safety, and environmental safety and participates in regular in-service education in this area.
• Demonstrates a commitment to customer service when dealing with patients, colleagues, volunteers and other members of the community and public.
• Demonstrated pattern of good attendance
