Position Overview:
Reporting to the Manager, Cardiac Specialty Clinics, the Heart Failure Navigator (Nurse Practitioner) is responsible for providing creative and innovative leadership ensuring the delivery of quality patient care within the cardiovascular program. The Heart Failure Navigator is responsible for facilitating integrated care through communication with strategic members of the interdisciplinary team, collaborate to provide staff education and works closely with patients, families and members of the health care team to optimize patient satisfaction and achieve optimal outcomes. The scope of the role involves providing administrative and clinical leadership within the SHN Cardiac Specialty Clinics. The Heart Failure Navigator will provide timely access to outpatient cardiac services at SHN and partner organizations. In this role, the Navigator aides in referral triaging, leads the CHF remote home monitoring program, and ensuring efficient access to cardiac outpatient services for chronic disease management at SHN.
As an integral member of the Cardiology program, the Heart Failure Navigator will work collaboratively with the multidisciplinary team including physicians, to ensure the efficient and effective delivery of patient care services within the program.

Responsibilities include the ongoing development of the patient care teams, and serves as the link between staff, the administrative leaders, physician leaders, and other clinical service support areas. Also, to support and implement evidence based, best practices and continuing process improvements to meet the department, program and organizational goals.

Requirements:
• Master′s Degree in Nursing and member of a regulated health care profession required.
• Current registration with College of Nurse of Ontario
• Certification in cardiology or related field is preferred
• Minimum 3 years clinical experience in Cardiovascular care and supports (i.e., Coronary care unit, inpatient cardiology units etc.), with demonstrated clinical and leadership skills
• Advanced knowledge, decision making skills and expertise in the nursing care of patients with cardiac issues
• Demonstrated competency in the following areas: health assessment and diagnosis, health care management and therapeutic intervention, health promotion and prevention of illness, injury and complications in the care of patients with cardiac issues
• Minimum of 2 years work experience as a Nurse Practitioner, preferred
• Demonstrated commitment to evidence informed practice and patient and family-centred care
• Effective and exceptional communication skills with internal (i.e. support services, other clinical programs) and external partners (i.e. Ontario Health Teams, Home and Community Care, CorHealth, Stroke Network etc)
• Demonstrated ability to take initiative and a commitment to continuing professional development
• Excellent interpersonal skills, including the ability to communicate effectively (written and verbal)
• Demonstrated team player and a strong ability to deal with a wide range of individuals tactfully and diplomatically
• Demonstrated strategic leadership, team building and conflict resolution skills
• Strong and excellent knowledge of best practices related to patient care and expectations
• Proven strong problem-solving, negotiating, planning and decision-making ability
• Demonstrates values consistent with SHN′s Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation
• Demonstrates an understanding of and commitment to SHN's comprehensive safety programs and practices including staff safety, patient safety, and environmental safety and participates in regular in-service education in this area

Asset:
• Ability to speak another language
• Experience with electronic medical records systems is a plus
• Experience in Case Management
• Previous experience in a cardiology clinic or similar setting
• CNA (Canadian Nurse Association) Certification in Cardiovascular Nursing
