Reporting to the Integrated Stroke Unit Manager, the successful Registered Practical Nurse candidate will be responsible for assessing, planning, implementing and evaluating nursing care and practice activities in a multidisciplinary person centered care environment. You will provide acute and rehabilitative care for patients following an acute stroke.

Requirements:

•Current certification with the College of Nurses of Ontario
•Competent to perform CPR at a Basic Cardiac Life Support level and evidence of continued education
•Demonstrated ability to carry out relevant clinical Best Practices, Knowledge integration, and skills within the management of complex stroke cases that bridge the full continuum of care
•Evidence of sound organizational and prioritization skills with the ability to work well in stressful situations
Demonstrated experience and proficiency in the provision of effective clinical consultation and care for the complex frail elderly
•Excellent communication, documentation, and interpersonal skills with the ability to work well with patients, families, all levels of staff and the community
•Facilitates application of relevant information to make effective judgments and decisions to lead the management of care.
•Ability to work well independently, exercise sound judgement, proven analytical thinking and proactive problem solving and decision making skills
•Ability to perform the essential duties of the position
•Demonstrated ability to work well collaboratively in a multidisciplinary team
• Demonstrates values consistent with SHN′s Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation
• Utilize protective equipment provided and identify and reports hazards
• Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community
• Strong work ethic and commitment is required
•Demonstrated high standards of work outputs
•Highly punctual
•Demonstrated professionalism and reliability

Asset:

• Graduate of an RPN diploma program
•CNA Rehabilitation nursing
•Neuroscience/Stroke Care Courses
•Health Assessment Course or equivalent experience
•Current experience in acute adult medicine and rehabilitation
•Current experience in Stroke/Neuroscience Nursing
