Reporting to the oncology pharmacy manager, this Full time 18 month temporary Pharmacist will be responsible for the clinical care of patients undergoing cancer treatment. This includes meeting the medication related needs of oncology patients, maintaining electronic patient records and drug profiles, providing education to patients, families and caregivers and positively contributing to maximize medication safety for cancer patients. Rotations include oversight of chemotherapy preparation, cancer medication order verification, pharmaceutical care patient monitoring related to injectable and oral take home cancer drugs, as well as projects as assigned. Cross site coverage between Centenary and General site oncology pharmacies and the General site oncology drug store as necessary. Other duties as assigned.

Requirements:

• Bachelor of Science in Pharmacy (B.Sc. Phm.) or Doctor of Pharmacy (Pharm D)
• Licensed and registered with the Ontario College of Pharmacists
• Previous experience in hospital pharmacy
• Demonstrated ability to exercise independent judgment and to assess situations and problems efficiently and effectively
• Individual must be highly motivated, personable, knowledgeable and possess excellent communication skills
• Patient oriented and patient focused. Demonstrates ability to handle sensitive issues with tact, diplomacy and confidentiality
• Ability to work well within an inter-professional team and to foster effective working relationships
• Ability to exercise initiative and observe strict confidentiality
• Demonstrated focus on internal/external customer service and partnerships
• Demonstrates values consistent with SHN′s Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation
• Utilize protective equipment provided and identify and report hazards
• Engage in ethical conduct and strive to understand and appreciate the diversity of our patient/staff population and community
• Excellent work performance and attendance
• Knowledge and demonstrated proficiency of computer software programs - including MS Office

Asset:

• Oncology pharmacy experience including oral take home cancer drugs, clinical monitoring and Cancer Care Ontario drug programs.
• Experience with IV Admixture sterile compounding including hazardous drugs.
• Retail pharmacy experience with Kroll in an oncology setting
• Hospital Residency experience
• Strong understanding of drug coverage in Ontario
• Experience using EPIC (Clinical Information System, Beacon and Willow modules)
• Ability to speak another language
