The Senior Pharmacist - Antimicrobial Stewardship Program Pharmacotherapy Specialist (ASP PS) is an integral member of the Antimicrobial Stewardship Program (ASP) and a practice leader in establishing corporate initiatives that foster appropriate and cost-effective antimicrobial use, reduce antimicrobial resistance, and enhance patient safety across the Scarborough Health Network.

The ASP PS will work with ASP and Pharmacy Services to establish guidelines that ensure appropriate antimicrobial therapy including: antibiotic prophylaxis, guidance for empiric antimicrobial therapy, optimizing antimicrobial therapy associated with a definite diagnosis, optimizing antimicrobial dose, route of administration and duration of therapy based on pharmacokinetic and pharmacodynamic principles.

In collaboration with ASP and other hospital departments/programs, the ASP PS will provide implementation strategies for initiatives to optimize antimicrobial use that may include: prospective and retrospective evaluation of ongoing compliance and provide feedback, in conjunction with the hospital′s continuous quality improvement initiatives. The ASP PS will assist ASP in antimicrobial resistance surveillance, antimicrobial use evaluation and reporting. The ASP PS will be responsible for providing education and feedback to pharmacists and physicians regarding appropriate use of antimicrobial agents supported by evidence-based medicine and best practice guidelines.

Requirements:

• Completion of a Bachelor′s degree program in pharmacy, or recognized equivalent.
• Advanced clinical degree such as a post-baccalaureate Doctor of Pharmacy degree.
• Active registration with the Ontario College of Pharmacists.
• Infectious disease training or equivalent clinical experience (e.g. pharmacy fellowship program).
• Minimum five years of relevant pharmacy experience.
• Self-motivated, can work independently and has excellent interpersonal skills to work effectively with inter-professional teams.
• Demonstrates leadership skills.
• Excellent verbal and written communication skill required.
• Strong critical thinking and critical appraisal skills.
• Excellent time management and organizational skills required.
• Knowledge of research, statistical and analytical methodologies.
• Strong computer skills and demonstrates understanding of health information systems and computer database management.
• Teaching experience.
• Able to work on complex problems with various clinical and technical stakeholders to provide innovative and creative solutions to achieve resolution and maintain patient safety.
• Demonstrates values consistent with SHN′s Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation.
• Utilize protective equipment provided and identify and reports hazards.
• Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community.
